School principals and department heads, or designated staff, must report injuries within 24 hours by phone to the District Safety Coordinator (323-4623 or 527-7803).

BPS Designated Medical Provider Form (doc document)

If you need medical care for a work related injury, you must go to a designated medical provider. If you go to someone else, Workforce Safety and Insurance may not pay for medical treatment unless it was an emergency. It will also make it harder for you to prove you have an injury that is covered by workers compensation.
