H1N1 Vaccination Clinic

Vaccination Priority Groups:
1. People ages 19 - 64 with chronic medical conditions.
2. All children 6 months through 18 years of age.
3. Second dose for children ages 9 and under if it's been 28 days since the first shot.
4. Pregnant Women
5. Household contacts and/or caregivers of infants younger than 6 months of age (parents and siblings)
· Please bring the Influenza Vaccination Record card showing the date of the first vaccine.
· A parent or guardian must accompany minor children under the age of 18 to provide vaccination consent.
DON'T COME TO THE CLINIC IF YOU HAVE ANY SYMPTOMS OF THE FLU!
· Vaccine will be given until supply is gone
· There will be NO CHARGE for this service
· Please obey posted parking signs.
